LoadingThe Continuity Passport
Forget the big system. The Passport is the record and the story that travel with your child, held by your family. Simple to make, simple to keep, and useful the moment it lands.
The Passport is one living page: your child's strengths, what helps them settle, where they are in their learning, and the one thing a new adult should know. It is held by your family, and you control every word.
It is built around what a new teacher can do, not a data dump. Strengths first, so it dignifies your child instead of labeling them, and it always names a real person to call.
A new teacher usually spends months learning who a child is. With the Passport, they know how to help on day one instead of month three.
A busy teacher will not open a database. They will read one page if it tells them, fast, how to help this specific kid this week. So that is exactly what the page is.
At intake, the Navigator and family fill one page together. Fifteen minutes. Strengths first.
The Navigator updates it as things change, especially right before a move. Nothing else has to be perfect, just this page.
Before or on day one at the new school, the Navigator gives the new teacher the page and introduces the child warmly. That is where the value is created.
A sample Passport
A design sample using an illustrative example named Maya. Nothing here is a real child.

“A quick, funny kid who warms up slowly and remembers everything you tell her.”
Maya has moved three times this year. She is not withdrawn, she is tired of starting over. Consistency from you will go a long way.
A warm welcome and one classmate buddy. Let her know you are glad she is here.

This page belongs to Maya and her family. Please use it to help, never to label. We never collect or share immigration status.
The official records travel with Maya too, released only under her family's consent, and only when a real need calls for them. The page above is what gets used. These are backup, not the product.
